Patient Portal
Access to your health records with Athens Pulmonary & Sleep Medicine are available through our online patient portal. This is a free service available to all of our patients. We encourage you to sign up for your free patient portal account and take advantage of the many convenient benefits offered by this system.

Medical Records Request
There are many reasons why you may need access to a copy of your medical records. In accordance with HIPAA guidelines, you have the right to request access to some or all of you medical records with Athens Pulmonary & Sleep Medicine. The request will be handled by our Medical Records Department and will involve the mandatory completion of certain HIPAA requirements.
If you need to request access to your medical records, please contact our Medical Records department to discuss your specific medical records request.
Billing Information
Athens Pulmonary & Sleep Medicine works with our patients to provide multiple methods for handling patient balances. Payments can be made in person, through the mail, over the phone, or online.
If you have questions about a patient balance, please call our office directly and ask for the Billing Department.
In order to make a payment via telephone or online, you must have an account statement with your unique billing code. For more information about online or telephone payments, click the link below.
Referral Information
Athens Pulmonary & Sleep Medicine is a specialty provider that requires patients to be referred to our office from an outside medical facility of provider. Many, if not most, insurance companies require a referral in order to cover medical claims incurred by a medical specialist. If you are a patient seeking a referral or are a Provider looking to refer a patient to our office, please use the Referral Form provided below.
